Section 8 of your grant contract requires that you acknowledge RAC's financial support in all printed and broadcast promotion, publicity, advertising and printed programs with the following credit line: “With support from the Regional Arts Commission” and the logo.
Operating Support End of Year Report
Grant Year 2011-12 (ending May 31, 2012) - the 2nd year of your 2-year grant.
Please fill out this form after May 31, 2011 and return it to our office by July 13th, 2012:

Invoices may be initially e-mailed or faxed to RAC to start the payment process, but we must receive an original, signed copy before we can release your payment.
