Logos

Section 8 of your grant contract requires that you acknowledge RAC's financial support in all printed and broadcast promotion, publicity, advertising and printed programs with the following credit line: “With support from the Regional Arts Commission” and the logo.

Grant Year 2007-08 (ending May 31, 2008) - the 1st year of your 2-year grant.
Please fill out this form after May 31, 2008 and return it to our office by June 30, 2008:

In order to receive your 2008-09 award, you must also submit the following by the June 30, 2008 deadline:

  • Current board list
  • Updated program / event schedule for 2008-2009
  • Audit – if you have not already done so, please submit an audit for your most recently completed fiscal year as soon as it is available

Invoices may be initially e-mailed or faxed to RAC to start the payment process, but we must receive an original, signed copy before we can release your payment.
